How Muscle Growth Actually Works

Muscle grows because hard contractions signal it to. The pump, the burn, the soreness and the hormone spike are things that happen alongside growth rather than causes of it, which is why progressive overload is the part worth organising your training around.

The short version

A muscle grows when its fibres are made to produce high force, repeatedly, more than they are used to. The fibres detect that mechanical stress directly, and the detection turns on the machinery that builds new muscle protein. Do that often enough, eat enough, and the muscle gets bigger.

Everything else you have heard about is either a consequence of training hard or a bystander.

Muscles sense force, not effort

The useful mental model is that a muscle fibre has stretch and tension sensors built into it. When a contraction is hard enough, those sensors trigger a chain of signalling inside the cell that ends with more muscle protein being made. The technical name is mechanotransduction, which just means turning mechanical force into a biological signal.

This is why a hard set works and a long walk does not, despite both being exercise. It is also why the signal is about force in the muscle rather than weight on the bar. A machine that puts 40 kg of tension through a muscle and a barbell that puts 40 kg through it are, to the fibre, doing similar things.

And it explains the single most useful fact in training: to keep growing, the demand has to keep rising. That is progressive overload, and it follows directly from how the sensing works.

What is not driving it

An evidence review of hypertrophy mechanisms went through the popular explanations one at a time (Lim et al., Medicine and Science in Sports and Exercise, 2022). Three came out badly.

The hormone spike. The idea that the testosterone and growth hormone rise after a big session drives growth has little support. The review found no support for acute changes in anabolic hormones being mechanistically responsible for hypertrophy. The clearest argument against it is not a lab result: women build muscle at a similar rate to men despite far lower testosterone. A theory that cannot explain half the population is not a good theory.

The burn. Metabolic stress, the accumulation of metabolites that makes a set feel like acid, was long thought to contribute. The review found little to no evidence for it, and made a sharp observation: endurance exercise produces similar metabolite concentrations and nothing like the growth.

Muscle damage. Soreness is a poor proxy for growth. The review calls damage a poor stand-in for muscle protein synthesis and hypertrophy, and points out that damage falls off as you get used to a movement while growth continues.

That last one deserves emphasis, because soreness is the measure most people use without noticing. Soreness tracks novelty. A new exercise makes you sore. It does not follow that it grew you more.

The pump

Worth its own paragraph, because it is the most enjoyable thing in the gym and the most misread.

The pump is fluid moving into the muscle during work. It is a genuine indicator that you have done a lot of contractions with limited rest. It is not itself a growth signal, and chasing it by picking lighter weights and shorter rests can trade real tension for a sensation.

Enjoy it. Do not steer by it.

What actually matters, in order

Given the above, the levers worth pulling are unglamorous.

  1. Hard sets, close enough to failure to matter. Somewhere in the last few reps is where the high-force contractions live. How close to failure to train covers where to stop.
  2. Enough of them per week. Volume is the dose. How many sets per week covers what "enough" looks like.
  3. More over time. Same sets, same weight, forever, means the same signal forever.
  4. Protein and calories. Signal without material does not build anything.
  5. Sleep and time. The building happens between sessions, not during them.

Nothing on that list is a technique. They are all quantities, which is why training well is mostly a bookkeeping problem.

What this changes about your training

Honestly, less than you might hope. Knowing the mechanism does not unlock a better routine, because the practical advice was already correct. What it does is stop you being sold things.

It tells you a workout that left you unable to walk was not necessarily better. It tells you the exercise that gives the best pump is not automatically the best exercise. It tells you that the supplement promising a testosterone boost is aiming at a mechanism that does not appear to do the job. And it tells you why the boring advice, add a little over time, is the whole game rather than a beginner simplification.

Why the bookkeeping is the hard part

If growth depends on doing gradually more hard work, then the thing you need is not motivation. It is a record of what you did last time, accurate enough to beat.

That is the argument for logging, and it is the only argument that matters. The mechanism is interesting. The implication is dull and correct: do a bit more than last time, repeatedly, for a long time.
